How To Know If Breast Augmentation Is Right For You

The typical candidate for breast augmentation will be dissatisfied with her breast size and have a strong desire to change it. Ideal candidates will be non-smokers, have a stable body weight and be capable of understanding the risks of cosmetic surgery.

Type of Implant

All breast implants have a silicone outer shell. The filler material can be of two types: saline or silicone. Safety of breast implants is often in the news and has been an important topic in plastic surgery for many years. As discussed previously, the FDA approved the use of silicone implants in breast augmentation in 2006 after years of study proved them to be safe. In fact, breast implants are some of the most rigorously investigated and researched medical devices available. The type of implant selected will be based on the discussion between the patient and our physician. Advantages and disadvantages exist for both.

Saline implants
Advantages:

  • Less expensive
  • Smaller incisions
  • Ruptures are easy to detect since the implant quickly deflates
  • Allows for more ease in placing different volumes in each breast since the implant is filled during surgery

Disadvantages:

  • Feels less natural
  • More prone to rippling

Silicone implants
Advantages:

  • More natural and softer feel
  • Less rippling compared to saline implants
  • Can give a less “augmented” look in certain patients with thin skin

Disadvantages:

  • More expensive
  • Requires larger incisions
  • Not FDA approved for patients under the age of 22

How Breast Augmentation Surgery Is Performed

Breast Implant TexasA variety of techniques exist for this procedure. All techniques currently being performed by plastic surgeons involve the placement of saline or silicone filled breast implants. The breast implants can be placed using three possible methods:

  • Submuscular – The breast implant is placed behind the pectoralis major muscle (large muscle of the chest) as shown in the below diagram on the left. The breast gland itself, lies on top of the muscle. In reality, the implant lies partially behind the muscle and partially behind the breast tissue. The advantages of this pocket include less risk of rippling, lower rate of capsular contractor, and less visibility and palpability of the implant. The downside is that the surgery can be slightly more painful and there can be implant movement with muscle contraction especially in athletic women.
  • Subglandular – The breast implant is placed directly behind the breast gland and on top of the muscle as shown in the diagram below on the right. The advantages of this pocket include allowing for a better “lift” of the breast and less pain after surgery. The disadvantages include more visibility of the implant and potentially higher rate of complications such as capsular contractor.

Breast Augmentation Incisions

Breast augmentation involves making a small incision to insert a breast implant into the breast area in order to enlarge the breast. There are several possible locations for the small incision that will be used for inserting the breast implant:

  • Inframammary – An incision is made at the base of the breast. It is the most common approach, especially in breast revision surgery.
  • Transaxillary – The incision is made in the armpit. This approach is less frequently used.
  • Periareolar – The incision is made around the areola (the darker skin surrounding the nipple).
  • Transumbilical breast augmentation (TUBA) – The incision is made around the belly button. This approach is rarely used and only saline implants can be placed via this approach.

Recovery Process

The surgery is commonly performed on an outpatient basis at a hospital or state-of-the-art surgical unit while the patient is under a general anesthesia and asleep. The surgery will take anywhere from 1 to 2 hours. All incisions are closed with absorbable sutures and covered with skin glue. This allows the patient to get in the shower the same day of surgery if she wishes. A surgical bra is placed at the end of surgery and the patient is instructed to wear this bra or any other bra without an underwire for a total of six weeks.

Postoperative instructions include no upper body exercises of any kind for a total of 6 weeks. Oral pain medications, muscle relaxants, and sometimes antibiotics are given as prescriptions for the postoperative period. Generally some pain is experienced for the first week, especially if the implants have been placed under the muscle. Swelling can persist for 4-6 weeks but is usually mostly resolved by two weeks. Sometimes, implants can “ride high” for the first few weeks. They will generally drop with the assistance of implant massage after 4-6 weeks. Most patients can have a good sense of their final result anywhere between 6 weeks and three months.
