Breast Augmentation


Women who are unsatisfied with the size or shape of their breasts may be interested in undergoing breast augmentation surgery. Breast augmentation can be performed to enlarge the breasts and to improve breast shape, and it is one of the most popular plastic surgery procedures. Many women who have undergone breast surgery have achieved better body proportions as well as a more defined and feminine silhouette.

Breast Augmentation Options

Implant Type

Saline:
  • A silicone shell filled with sterile saltwater
  • Allows for smaller incisions and easier placement
  • Implant ruptures are easily detected
Silicone:
  • A silicone shell filled with a soft, elastic gel that comes in a variety of projections
  • Provides a natural feel, less chance of implant rippling, and a less “augmented” look
Teardrop (“Gummy Bear”):
  • A textured silicone shell filled with a cohesive gel made of crosslinked silicone molecules
  • Anatomically shaped to mimic the natural breast slope
  • In comparison to traditional implants, teardrop implants are firmer and will hold their shape in the case of implant rupture

Implant Placement

Submuscular:
  • Placed behind the pectoralis muscle (large muscle of the chest between the breast tissue and ribs)
  • The implant will lie partially behind the muscle and partially behind the breast tissue
  • Offers a reduced risk of rippling, a lower rate of capsular contracture, and less visibility and palpability of the implant
Subglandular:
  • Placed directly behind the breast gland and above the pectoralis muscle
  • Provides a little bit of a lift to the breast tissue
  • Easier recovery compared to submuscular implant placement

Incision Location

Inframammary:
  • An incision is made at the base of each breast
  • The most common approach
Transaxillary:
  • An incision is made within the natural crease of each armpit
Periareolar:
  • An incision is made around each areola (the darker skin surrounding the nipple)
Transumbilical (TUBA):
  • An incision is made inside the belly button
  • Limited to augmentation with saline breast implants

Recovery

Pain and discomfort are common, especially within the first 48 hours. At the end of surgery, a surgical dressing or surgical bra will be placed over the breasts to help with bruising and swelling. Patients will need to wear the surgical bra or a bra without underwire for at least six weeks. Swelling can last for four to six weeks. The breasts will feel tight and sensitive to the touch, and the skin might feel warm or itchy. For the first few weeks, the implants may “ride high” on the chest. Patients may begin an implant massage after one week if they have a certain type of implant, which will help the implants to drop into their desired position.
Patients are instructed to avoid performing upper body exercises, such as lifting, pulling, or pushing, for at least six weeks following surgery. Most patients can return to work within a few days to a week and resume physical activity in about two weeks.
Oral pain medications and/or muscle relaxants can be prescribed to help ease any pain or discomfort from bruising and swelling. Antibiotics should be taken following surgery to prevent infections.
The extent of scarring will depend on the incision location and the patient's ability to heal. Incisions will be closed with absorbable sutures and covered with skin glue, allowing for the patient to shower the same day of surgery. Throughout the recovery process, which can last up to three months, most scarring will fade and become even less noticeable over time.

Breast augmentation results are relatively permanent; however, the breasts can change due to weight fluctuations, aging, and pregnancy.

In cases where a patient has breast sagging, a breast lift may be performed with a breast augmentation to lift the breasts into a higher, more youthful position. Often, breast augmentation is combined with other body contouring procedures, such as tummy tuck surgery and liposuction.
